At the Lamb's Chapel it is our goal to share the Gospel in an exciting, tangible, uplifting environment. We strive to not only teach children about the Lord, but equip children to live their faith and share their faith to a lost generation. We believe that children are important to the kingdom of God. God wants to use children to change the world for His sake.

- Medical Release Form
- Must be completed notarized prior to participating in any church sponsored event away from the church property. Form is good for one year
